If your dashmoto® scooter is purchased and used as a mobility aid, you may qualify for a sales tax exemption with a valid prescription from your doctor.

To begin the process, download the tax exemption form below, complete and sign it, then submit it along with your prescription for review. Once received, our team will guide you through the next steps.

For the smoothest experience, we recommend submitting your exemption form before purchase so our team can issue a tax-adjusted invoice. If you have already completed your purchase, you may still submit the form and prescription to request a refund of eligible sales tax.
